Instances in which information will be disclosed include:

• Information being subpoenaed by a court.
• Failure to disclose information would place you or another person at risk.
• Your prior approval has been obtained to provide a written report to another professional or agency, (e.g. medical practitioner, lawyer, Workers Compensation), or to discuss the material with another person (e.g. a parent or employer or other third-party billing source).
